I would like copies of communications that pertain to incidents in which T/Os opened train doors manually on the wrong side (non-platform side) of the train or when the train was not properly berthed in the platform. I would also like copies of bulletins or emails that went out to try to prevent these incidents from occurring. Finally, I would like emails/bulletins (likely to train controllers/OCC personnel) that put in place the current procedure for authorizing yard manual door cycles. (T/O requests manual door cycle, confirms that the train is berthed properly, confirms that the doors will be opened on the platform side only, then gets authorization). This process takes up an excessive amount of time, frustrates patrons, causes delays, and ties up the radio from emergency transmissions.
